Job summary

Mountainvalleycares in Elkin, NC is looking for an Evening Home Care RN to provide skilled nursing care to hospice patients during evening hours (Monday–Friday, 4 PM–Midnight). You'll monitor patient conditions, communicate changes, and support families through education.

As an Evening Home Care RN, you'll provide skilled nursing care, comfort, and support to hospice patients and families in their homes during evening hours. This role is ideal for an RN who enjoys working independently, building meaningful relationships, and making a real difference in the lives of others.

Schedule: Monday–Friday, 4:00 PM–Midnight

Responsibilities

• Provide skilled nursing care to hospice patients.
• Monitor patient conditions and communicate changes to the care team.
• Administer medications and treatments as ordered.
• Support patients and families through education and guidance.
• Document care accurately in the electronic medical record.
• Collaborate with the interdisciplinary team to ensure exceptional patient care.

Qualifications

• Current RN license in North Carolina.
• One year of nursing experience preferred.
• Hospice, home health, or long-term care experience preferred.
• Strong communication and critical thinking skills.
• Valid driver's license and reliable transportation.
