Job summary

AdventHealth Hospice Care in Florida seeks an experienced RN to assess hospice eligibility and coordinate patient care across hospital and home settings. You will travel to patient locations, collaborate with account representatives, and ensure proper documentation for reimbursements.

The role requires 3+ years RN experience, active RN license and BLS/CPR, with a focus on home health and hospital care. Travel and scheduling options are provided to meet patient needs.

Qualifications

• 3+ years RN experience in home health and/or acute care; includes 1 year in hospital
• RN license and BLS/CPR required
• Driver's license required
• Knowledge of EMR and MS Office; infection control knowledge
• Associate's degree in Nursing required; Bachelor's preferred

Responsibilities

• Travels to patients' locations to assess hospice eligibility and communicates services available
• Partner with Account Representatives to meet patient/hospital staff needs for referrals
• Obtain orders and initiate documentation for crisis care and reimbursement
• Evaluate patients' medical needs, current medications, and equipment requirements
• Notify Hospice team for crisis intervention and coordinate care
• Arrange patient transportation as needed and ensure continuity of care

Job Description

Sign On Bonus For eligible applicants, up to $10,000

Schedule: Hospitals 10am-630pm Monday-Friday plus 2 weekend days/month.

2nd Schedule Option: Field Based 11:30am-10pm with every other weekend

3rd Schedule Option: Every Weekend 10am-8:30pm - Every Weekend plus 2 variable days/wk. Weekend Bonus Paid.

Travel: Covers Orange, Osceola and Seminole Counties

Travels to patients' locations to assess their physical and emotional condition for hospice eligibility and communicates program and services available. Partners with Account Representatives to meet patient and hospital staff needs for new referrals. Obtains required signatures and complete necessary documentation to comply with regulations for reimbursement. Evaluates patients' medical needs, current medications, and special equipment requirements. Notifies Hospice team members for crisis intervention or special services as needed. Obtains orders and initiates documentation for crisis care if required. Obtains approval from Hospice and primary care physicians to admit patients to the program. Updates referral sources on the status of admissions promptly. Documents and enters patient information into the electronic medical record in a timely manner. Notifies the patient care team of admissions via electronic mail to ensure continuity of care. Arranges patient transportation as needed based on circumstances. Communicates with applicable department(s) and Pharmacist regarding patient equipment and medications. Travels to current hospice patients hospital rooms who have been admitted. Knowledge, Skills, And Abilities

• Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
• Knowledge of electronic medical record. [Required]
• Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]
• Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software. [Required]
• Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form. [Required]
• Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]
• Ability to complete AdventHealth Hospice Care CFL Orientation and Training, as necessary. [Required]

Work Experience

• 3+ years registered nurse experience in home health and/or acute care nursing experience; including 1-year experience in the hospital [Required]
