Job summary

Lakeland Nursing and Rehab in Wanaque, New Jersey, is hiring Registered Nurses (RNs) to provide compassionate, resident-centered care on all shifts. You’ll assess, plan, implement, and evaluate care plans, administer medications, and collaborate with the care team to ensure holistic services.

Ideal candidates hold an active New Jersey RN license with 1–2 years SNF experience, strong communication, organization, and critical thinking, and a commitment to quality improvement and patient advocacy.

Qualifications

• Active RN license in New Jersey.
• 1–2 years SNF (Skilled Nursing Facility) experience.
• Excellent communication, organization, and critical thinking abilities.

Responsibilities

• Provide direct patient care to residents, including assessing, planning, implementing, and evaluating care plans.
• Administer medications, perform wound care, and conduct vital signs as needed.
• Collaborate with other members of the healthcare team to ensure comprehensive care.
• Develop and implement educational programs for residents and families.
• Communicate effectively with residents, families, and healthcare team members.
• Maintain accurate and detailed records of patient care and treatments.
• Participate in quality assurance and improvement activities to ensure high-quality care.
• Foster a positive, team-oriented, and collaborative work culture.
• Contribute to quality assurance initiatives, budget planning, and internal committees.
• Ensure accurate MDS documentation and coordinate discharge planning with families.
• Provide individualized, compassionate care to residents and their loved ones.
