Job description

The Registered Nurse in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. The RN coordinates care plans with healthcare professionals and ensures continuity of care by exchanging information with physicians, patients, families, and community resources, all within a Point of Care setting.

Position Details:

  • Full-time, pay range $90K - $110K depending on experience.
Responsibilities:
  1. Clinical Competence: Develop and revise care plans in collaboration with physicians and healthcare team.
  2. Supervision: Supervise home health aides and nurses, provide instruction, and perform supervisory visits as per regulations.
  3. Documentation and Care Delivery: Complete assessments, medication reconciliation, and document visits accurately and timely.
  4. Quality: Conduct evaluations, ensure patient eligibility, develop care plans, and communicate effectively with team members and physicians.
  5. Professionalism: Maintain punctuality, participate in training, and demonstrate a positive attitude.
  6. Teamwork: Follow directives, assist in onboarding, participate in performance improvement, and adapt to schedule flexibility, including weekends and holidays.
  7. Utilization Management: Adhere to agency policies and participate in management activities.
License Requirements:
  • Current RN licensure in the practicing state.
  • Current CPR certification.
  • Valid driver’s license, vehicle insurance, and access to transportation.
