Job summary

Independence Health and Rehabilitation Center in Independence, Oregon, is seeking a compassionate RN to join a highly collaborative care team supporting skilled nursing, rehab, and long-term care as patients transition from hospital to home.

You will provide direct patient care, develop care plans with an interdisciplinary team, assess changing needs, and work closely with nurses, therapists, and providers to achieve optimal outcomes.

Qualifications

• Active Oregon RN license.
• Experience in skilled nursing, rehab, or long-term care preferred.
• Strong teamwork and communication skills.
