Ideal candidates will have an unrestricted Michigan RN license, BLS, 2+ years in clinical nursing with case management, and comfort with telehealth platforms and EHRs.

Qualifications

• Current unrestricted RN license in Michigan.
• BLS Certification required.
• Minimum 2 years of clinical nursing experience, with case management preferred.
• CCM certification preferred.

Responsibilities

Job Summary

As a Telehealth RN Case Manager, you will work closely with patients, healthcare providers, and other stakeholders to ensure continuity of care, promote patient education, and facilitate positive health outcomes. This role requires strong clinical knowledge, excellent communication skills, and the ability to leverage technology to deliver exceptional patient care remotely.

Responsibilities

Requirements

• Current unrestricted RN license in Michigan.
• BLS Certification required
• Minimum 2 years of clinical nursing experience, with a background in case management preferred.
• Certification in Case Management (CCM) preferred.
• Proficiency in using telehealth platforms and electronic health records (EHR) systems.
• Excellent communication skills, both verbal and written.
