Responsibilities

• Perform assessments and identify all problems that affect health and wellness.
• Assist patients telephonically to create and implement person-centered care plans.
• Ensure compliance with all clinical documentation requirements.
• Complete medication reconciliation and coordinate with specialists.
• Ensure closure of the referral loop.
• Eliminate barriers to care through case management.
• Assist patient with scheduling appointments with physicians.
• Connect the patient to necessary resources to achieve health.
• Conduct Interdisciplinary Care Team Conferences.
