Job summary

Sciometrix in Royal Oak, MI is seeking a highly skilled RN Case Manager to join our telehealth-enabled care team. This onsite role supports remote patient care coordination from 8 am to 5 pm, Monday to Friday, with a focus on continuity and quality outcomes.

You will assess, plan, monitor, and educate patients, collaborate with a multidisciplinary team, and document all care activities in our EHR. A strong clinical background and compassionate patient focus are essential.

Qualifications

• Current unrestricted RN license in Michigan.
• BLS certification required.
• Minimum 2 years of clinical nursing experience, with case management preferred.
• Proficiency with telehealth platforms and EHR documentation.
• CCM certification preferred.

Responsibilities

• Conduct initial assessments and develop care plans for remote patients.

Job Summary:
