Job summary

Meadow Creek Post-Acute is a 104-bed Skilled Nursing Facility in Paramount, CA. We are seeking an experienced RN to supervise our clinical team on a full-time basis, overseeing LVNs and CNAs to deliver care per care plans.

The RN will manage staffing, monitor new staff, and maintain medical records while ensuring high-quality resident care. Active CA RN license and CPR/BLS are required.

Qualifications

• Active CPR/BLS certification.
• Skilled Nursing Facility experience preferred.
• SNF/Long-Term Care setting experience.

Responsibilities

• Oversee LVNs and CNAs in providing direct patient care per each patient’s care plan.
• Manage area to achieve the highest quality of care possible.
• Review staffing patterns and census; reassign personnel to meet resident needs and budgetary controls.
• Monitor skills of new staff and guide those needing assistance with procedures.
• Supervise, direct, and evaluate assigned staff (LVNs and CNAs).
• Maintain accurate medical records, care plans, and progress notes; document interventions and resident responses.

Meadow Creek Post-Acute is a 104-bed Skilled Nursing Facility located in central Paramount, CA. Our dedicated, professional staff is equipped with strengths and talents to deliver the best possible care on a consistent basis.

We are currently searching for an experienced RN to supervise our clinical team including LVN’s and CNA’s. The ideal candidate will be able to work on a Full-Time basis.

Pay Range:

• $48-55/hour Depending on Experience

About the Position:

As the RN, your primary responsibility is to oversee LVN’s and CNA’s in providing direct patient care based on each patient’s care plan to the highest quality.

• Manages area of responsibility with the goal of achieving and maintaining the highest quality of care possible.
• Reviews staffing patterns and the census of nursing units and reassigns personnel, when necessary, to ensure staffing meets resident needs and budgetary controls.
• Monitors the skills of new staff members and guides and observes staff that may need additional assistance with procedures.
• Supervise, direct, and evaluate assigned staff members, LVNs, and CNAs.
• Initiates investigations of accidents and unusual occurrences and makes the necessary written report to the DON.
• Keeps the DON informed about the status of residents, staff, and related matters.
• Maintains accurate and up-to-date medical records, including nursing assessments, care plans, and progress notes.
• Documents interventions, observations, and resident responses to treatment.

Qualifications:

• Active CPR/BLS
• Skilled Nursing Facility experience preferred
• SNF/Long-Term Care Setting experience
