Job summary

Meadow Creek Post-Acute, a 104-bed skilled nursing facility in Paramount, CA, is seeking a dedicated CNA for a full-time role. You will support residents with daily living activities and nursing care per care plans, collaborating with the nursing team to ensure high-quality resident care.

The ideal candidate has at least 1 year of CNA experience in a skilled nursing setting, holds an active CA CNA certificate, and BLS/CPR certification.

Qualifications

Responsibilities

• Assist residents with ADLs: bathing, grooming, dressing, toileting, feeding.
• Take and record vital signs (BP, temp, pulse, respirations).
• Aid mobility and safe transfers using gait belts and lifts.
• Maintain a clean, safe, comfortable environment.
• Observe changes in residents and report to nurses.
• Document care and observations per policy.
• Collaborate with the nursing team.

The The ideal candidate has a minimum of 1 year of experience as a CNA working in a skilled nursing facility/subacute unit. This role will be on a full time basis.

Pay Range:

• $19-22/hour Depending on Experience

Position Overview:

As a CNA, you will support your assigned residents with routine daily activities and nursing care services in accordance with the resident’s assessment and care plan, and as may be directed by the supervisors.

• Assist residents with Activities of Daily Living (ADLs): bathing, grooming, dressing, toileting, feeding
• Take and record vital signs (blood pressure, temperature, pulse, respirations)
• Help with mobility and safe transfers using gait belts, Hoyer lifts, and other devices
• Maintain a clean, safe and comfortable environment
• Observe and promptly report any changes in a resident’s physical or emotional condition
• Collaborate with nurses and interdisciplinary team members as needed
• Accurately document care and observations according to facility policy and regulations

Qualifications:

• Highschool diploma or GED
• Active CNA Certificate in CA
• BLS/CPR
• Must have 1 year of experience as a CNA
