The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies.

• Coordinates the total plan of care with other healthcare professionals and helps maintain continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources, all within a Point of Care setting.
• Performs initial and/or comprehensive nursing evaluations, determines the primary focus of care, develops the plan of care within state-specific guidelines, and submits accurate documentation in collaboration with physician orders.
• Conducts comprehensive assessments at designated times and develops appropriate plans of care.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by the payer source.

License Requirements:

• Current RN licensure in Florida
• Current CPR certification
• Valid Driver’s License, vehicle insurance, and access to reliable transportation
