Job summary

Affirmed Home Care is seeking a per diem Pediatric Nurse Care Manager to oversee a New Jersey caseload in Mercer, Middlesex, Monmouth and Ocean counties. This 75% field-based role emphasizes autonomy and high-quality, personalized care in a home setting.

Requirements include NJ RN license, BSN preferred, BLS, and at least 2 years in pediatric home care with hands-on skills in vents, trachs, G-tubes, IVs, and wound care. Equal Opportunity Employer.

Qualifications

• Current NJ RN license in good standing.
• BSN preferred.
• Valid BLS Card.
• Minimum 2 years of recent pediatric clinical home care experience.
• Recent hands-on experience with advanced clinical skills including pediatric ventilator management, tracheostomy care, G-tube/PEG tube maintenance, IV infusions, and wound care.
• Exceptional assessment, communication & time management skills.
• Proven ability to work independently and manage field staff.
• Attention to detail and a client-first mindset.

Responsibilities

• Ensuring the highest quality of client care.
• Developing care plans.
• Obtaining necessary physician orders for home care cases as required by State regulations.
• Reporting changes in the client’s medical or mental condition to the attending physician and the Agency’s team involved in the client’s care.
• Supervising and supporting the home care team.
• Conducting nursing orientations and competency assessments.
• Striving to improve the lives and sustainability of our clients by supporting their autonomy in the home environment.
