Job summary

Affirmed Home Care in New Jersey is seeking a per diem Pediatric Nurse Care Manager to oversee a pediatric caseload across Mercer, Middlesex, Monmouth and Ocean counties. This field-based, autonomy-driven role emphasizes delivering personalized, gold-standard care in the home setting.

You bring NJ RN licensure, BLS, and at least 2 years of pediatric home care experience; BSN is preferred. Per diem schedule with competitive visit-based compensation supports flexible, patient-centered care.

Qualifications

• Current NJ RN license in good standing.
• BSN preferred.
• Minimum 2 years of recent pediatric clinical home care experience.
• Hands-on experience with pediatric ventilator management, tracheostomy care, G-tube/PEG maintenance, IV infusions and wound care.

Responsibilities

• Ensuring the highest quality of client care.
• Developing care plans.
• Obtaining necessary physician orders for home care cases as required by State regulations.
• Reporting changes in the client’s condition to the attending physician and the Agency’s team.
• Supervising and supporting the home care team.
• Conducting nursing orientations and competency assessments.
• Striving to improve the lives and sustainability of clients by supporting their autonomy in the home environment.
