Job summary

Aroostook Home Health Services is seeking a Registered Nurse to provide high-quality home health care for clients in the Houlton, Maine area. The RN will perform assessments, educate patients, coordinate care, and ensure safe, effective home-based treatment plans.

Responsibilities include medication administration, wound care, blood draws, and documentation, with an emphasis on patient independence and safety in the community setting.

Qualifications

• Graduate of an approved nursing program with Maine state license in good standing.
• Ability to relate well with patients, families, and professionals.
• Capable of working independently in home settings and making sound clinical decisions.

Responsibilities

• Perform comprehensive assessments and observations in the home setting.
• Record patient medical history and current symptoms.
• Administer medications and treatments as needed.
• Provide wound care and manage common health conditions.
• Coordinate care plans with the client’s care team.
• Conduct blood draws and urine sample collections as required.
• Educate clients and care teams on health conditions and daily management.
• Maintain accurate records per agency policies and ensure QA participation.

Position Summary:

The registered nurses provides essential services to clients living in the community setting. They provide comprehensive assessment, education, and follow-up to keep them as safe and independent as possible at home. The RN plays in integral role in their overall health and well-being and works collaboratively with other members of the interprofessional team to provide and coordinate direct care for our clients.

Essential Functions:

• Performing comprehensive assessments/observations
• Recording client's medical history & symptoms
• Administering medications/treatments when needed
• Wound Care
• Coordinating with the client's care team to ensure there is a correct Care Plan in place
• Blood Draws
• Urine Samples
• Educating the client & care team about the health condition(s) & how to manage on a daily basis
• Maintain records and documentation as per agency's policies.
• Participate in quality assurance, meetings, and in-service education as outlined in policies.
• Work within the scope of policies and procedures established by the organization as well as other professional and regulatory standards.

Requirements

• Graduate of an approved nursing program with a valid Maine State license in good standing.
• Has ability to relate well with the public, colleagues and other professional agencies.
• Able to work alone in home situations, use good judgements and demonstrate decision making skills in critical or emergency situations.
• Must have reliable transportation and be willing to travel to client's homes on assignments. Vehicle must be insured as per policy.
