White Point Care Center LLC in Los Angeles County is seeking a compassionate Registered Nurse (RN) for the Night shift (11PM-7AM) to provide skilled nursing care to residents in a SNF setting. You will administer medications, monitor conditions, document changes, and collaborate with physicians, therapists, and the care team to meet individual goals.

You must hold a CA RN license and have at least 1 year of SNF, acute care, rehab, or behavioral health experience.

Formación

• Valid and current RN license in California.
• Graduate of an accredited nursing program.
• Minimum of 1 year experience in a skilled nursing facility, acute care, or inpatient rehab/behavioral health.
• Strong clinical assessment, planning and implementation of nursing care.

Responsabilidades

• Provide direct nursing care, administer meds and monitor vital signs.
• Develop and implement individualized care plans based on resident needs.
• Coordinate care with physicians, therapists and healthcare team.
• Ensure documentation and charting are accurate and up to date.
• Assist with ADLs and educate residents and families about healthcare needs.
• Respond to emergencies and provide first aid as needed.
• Adhere to facility policies, procedures and regulations.
• Utilize PCC or similar clinical software for documentation.

Registered Nurse (RN) - White Point Care Center LLC

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at White Point Care Center, a Skilled Nursing Facility in Los Angeles County, California. This is for NOC shift (11PM-7AM). As an RN, you will be responsible for ensuring that residents' needs are met in accordance with professional standards of practice through physician orders, facility policies and procedures, and federal, state, and local guidelines.

Responsibilities:

• Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their condition

• Develop and implement individualized care plans based on residents' needs and goals

• Coordinate care with other members of the healthcare team, including physicians, therapists, and other healthcare professionals

• Ensure that all documentation and charting is accurate, complete, and up-to-date

• Assist with activities of daily living (ADLs) as needed

• Educate residents and families about their healthcare needs and treatments

• Respond to emergencies and provide first aid as needed

• Adhere to all policies, procedures, and regulations governing nursing care in the facility

Qualifications:

• Valid and current RN license in the state of California

• Graduate of an accredited nursing program

• Minimum of 1 year experience in a skilled nursing facility, acute care setting, or in-patient rehabilitation or in-patient behavioral health setting

• Strong clinical skills, including assessment, planning, and implementation of nursing care

• Excellent communication and interpersonal skills

• Proficient in using our clinical software Point Click Care (PCC) or similar
