Trinity Health At Home in Livonia, MI is seeking a Homecare Registered Nurse for day shift to provide primary nursing care in patients' homes, and to work with the Population Health Case Manager to develop individualized care plans and ensure high-quality outcomes. Responsibilities include admission assessments, care plan development, therapeutic interventions, health coaching, and timely documentation.
