Job summary

Trinity Health At Home is seeking a Homecare Registered Nurse to provide compassionate, high-quality care in patients' homes on a part-time day shift. You will assess patients, develop care plans with physicians, educate patients, and support independent health outcomes.

This role requires an active RN license in the state, a BSN is preferred, and mileage reimbursement is offered along with premium pay and bonus opportunities. Prior home health experience is a plus.

Employment Type: Part time Shift: Day Shift Description: Premium Pay + Bonus Opportunities and Mileage Reimbursement! Position Purpose Join Trinity Health at Home and make a meaningful impact by providing compassionate, high-quality care to patients in the comfort of their homes. As a Homecare RN, you will deliver primary nursing care as ordered by the physician, develop individualized care plans in collaboration with the Population Health Case Manager, and ensure exceptional patient outcomes and satisfaction. This position offers a competitive hourly rate with an additional premium for flexibility. Flex nurses may also be eligible for assignment completion bonuses when meeting productivity and attendance criteria, with increased bonus opportunities during peak months.

What You Will Do

Minimum Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current RN licensure in the state of practice.
• Minimum one (1) year of professional nursing experience; home health experience preferred.
• Strong computer and technology skills.
• Current driver's license and reliable transportation.
