Job summary
The Vineyards Healthcare Center in Livermore, CA is seeking a dedicated Registered Nurse to join our skilled nursing facility team. You will assess residents, plan individualized care, administer medications, and collaborate with interdisciplinary teams to ensure high-quality care.
We welcome fresh or newly licensed RNs and prefer experienced SNF nurses. A CA RN license, active BLS and CPR certifications, and flexibility to work weekends/holidays are required.
Qualifications
- Freshly licensed RNs welcome.
- Experienced RNs preferred.
- Experience in a skilled nursing facility/long-term care a plus.
- CA RN license required.
- Active BLS through the American Heart Association.
- Active CPR certification.
- Interest in working with the geriatric population.
- Ability to work in a fast-paced environment and manage multiple priorities.
- Willingness to work flexible hours, including weekends and holidays.
Responsibilities
- Conduct thorough assessments of residents' health status and needs.
- Develop and implement individualized care plans based on assessments.
- Administer medications and treatments as prescribed by physicians.
- Monitor residents' progress and adjust care plans as needed.
- Provide education to residents and families about health conditions and care options.
- Collaborate with interdisciplinary teams to ensure optimal care.
- Document all aspects of patient care accurately in electronic records.
