Job description

Purpose and Scope:

The registered professional nurse Home Therapies RN CAP 1 is an entry-level position within the Clinical Advancement Program (CAP). It is responsible for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for assigned patients. The nurse assesses and manages patients' responses to home dialysis training and treatments, following protocols and communicating issues to physicians as needed. As part of the End Stage Kidney Disease (ESKD) team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Principal Duties and Responsibilities:

All duties are performed in accordance with Fresenius Kidney Care policies, standards, and regulations.

1. Perform duties under supervision, with guidance from educators or preceptors.
2. Collect and analyze dialysis data, document findings, and adjust treatment plans accordingly.
3. Assess and document patient/family learning needs and provide education.
4. Coordinate safe dialysis therapy delivery with patients, home care partners, and staff.
5. Administer medications as prescribed, documenting justification and effectiveness.
6. Assist with emergency response measures.
7. Serve as a resource, participate in staff training and orientation.
8. Ensure proper laboratory procedures are followed and reschedule missed collections.
9. Update nursing assessments and care plans collaboratively.
10. Inform patients about transplant and treatment options.
11. Identify and educate Home Dialysis candidates and partners.
12. Assess home environment and partner readiness for dialysis.
13. Train patients and partners on dialysis equipment and supplies through a structured program.
14. Complete CAP requirements for advancement.
15. Perform other duties as assigned.

Physical Demands and Working Conditions:

This role involves heavy lifting, equipment movement, and prolonged standing. It may require moving equipment up to 200 lbs. and lifting solutions of up to 30 lbs. The environment is a healthcare facility with potential exposure to infectious materials. Coverage rotates among staff, and participation in on-call, night, weekend, and holiday shifts is required. Travel to training and patient locations may be necessary.

Supervision:

Oversight of LPNs/LVNs, RNs, and other staff as a Team Leader or Nurse in charge, following successful completion of training and experience requirements.

Education and Licensure:

Graduate of an accredited Nursing School, current state license, CPR/BLS certification, and compliance with state practice requirements.

Experience and Skills:

Entry-level for RNs with less than 2 years of nephrology nursing, with preferred experience in home dialysis therapy.
