Job summary

Signature HealthCARE Owensboro is seeking an experienced Registered Nurse (RN) to join our Hermitage, Hillcrest and Riverside teams in Kentucky. You will provide compassionate care, manage medications, and collaborate with an interdisciplinary team to support residents and their families.

We value strong clinical judgment, supervisory experience, and proficiency with EHR systems and MS Office. A valid RN license and CPR/BLS are required or preferred, with long-term care experience encouraged.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and MS Office.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort and safety.
• Supervise nursing assistants and medication aides during the shift according to policy.
• Monitor residents' condition and report changes to physician and families.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance activities and staff education.

As an RN, you’ll provide compassionate nursing care, support residents and their families, and collaborate with your healthcare team to deliver quality care.

How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.


• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.


• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.


• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.


• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.


• Complete accurate and timely clinical documentation per facility policy and procedure.


• Coordinate resident admissions, transfers, and discharges according to established procedures.


• Participate in quality assurance activities, staff education, and regulatory surveys as needed.


• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference:

• Current, active RN license in the state of practice.


• CPR/BLS certification preferred (American Heart Association or American Red Cross).


• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.


• Experience in a supervisory or charge nurse role, preferred.


• Strong clinical observation, medication administration, and documentation skills.


• Knowledge of Federal and State long-term care regulations.


• Effective verbal and written communication skills.


• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
