Job summary

Signature HealthCARE is hiring Registered Nurses (RN) to join our Hermitage, Hillcrest and Riverside facilities. You will deliver direct nursing care, supervise nursing assistants, and collaborate with the care team to ensure high-quality outcomes for residents.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong observation, medication administration and documentation skills.
• Familiarity with regulatory long-term care requirements.
• Proficient with EHR systems and basic MS Office.

Responsibilities

• Provide direct nursing care to residents, including meds and wound care.
• Assist with ADLs, ensuring dignity, safety and comfort.
• Supervise nursing assistants and aides during shifts.
• Monitor residents and report changes to physician/guardian.
• Develop and review individualized care plans with the team.
• Maintain accurate clinical documentation as per policy.
• Coordinate admissions, transfers and discharges.
• Participate in quality assurance and staff education.

Signature HealthCARE is seeking LPNs at our Hermitage, Hillcrest, and Riverside locations. As an RN, you'll provide compassionate nursing care, support residents and their families, and collaborate with your healthcare team to deliver quality care.

How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents' physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference:

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
