Job summary

Anew Home Health in the Indianapolis area is seeking a Registered Nurse to provide comprehensive in-home patient care across communities including Westfield, Noblesville, Carmel, Fishers and Zionsville. The RN will perform assessment, planning, implementation, and evaluation using the nursing process, focusing on community health and education.

Indiana licensure is required; home health and supervisory experience are preferred.

Qualifications

• Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.
• One (1) year of home health care experience preferred.
• One-year supervisory/management experience with direct care staff preferred.
• Excellent observation, verbal and written communication skills, problem solving skills; basic math skills; nursing skills per competency checklist.

Responsibilities

• Deliver patient care through the nursing process: assessment, planning, implementation, and evaluation.
• Plan, organize and direct home care services leveraging community resources.
• Provide nursing care with emphasis on community health education and family-centered services.

The Registered Nurse is responsible for the delivery of patient care through nursing process of assessment, planning, implementation, and evaluation. The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Job Requirements

• Graduate of an accredited school of nursing with RN licensure in the state in which services will be provided.

• Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies

  to perform required nursing skills.
