Job summary

Ideal Home Health Agency is seeking a compassionate, dependable Part-Time Registered Nurse to join our home health clinical team. The RN will complete admissions, recertifications, supervisory visits, and provide clinical oversight of Home Health Aides, while delivering individualized patient care at home.

This role offers a flexible schedule, local travel to patients' homes, ongoing clinical support, paid orientation and training, and collaboration with physicians, caregivers, and agency staff.

Qualifications

• Current, active, and unrestricted Pennsylvania RN license.
• Minimum one year of nursing experience; home health preferred.
• Strong assessment, clinical judgment, documentation and communication skills.
• Ability to work independently and manage a flexible patient schedule.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Ability to meet background checks and pre-employment requirements.

Responsibilities

• Complete Start of Care/Admission assessments, recertifications, supervisory visits and other required nursing visits.
• Assess patients' medical, functional, psychosocial, and environmental needs.
• Develop, review and update Plans of Care per physician orders.
• Conduct Home Health Aide supervisory visits and coach caregivers.
• Monitor patient condition and report significant changes to physicians and staff.
• Educate patients and caregivers on medications, safety, infection prevention and the Plan of Care.
• Complete accurate clinical documentation per requirements.
• Coordinate care with physicians, caregivers, office staff, and interdisciplinary team.
• Review caregiver compliance with the Plan of Care and address concerns.
• Participate in chart reviews, quality improvement, compliance activities and trainings.
• Maintain patient confidentiality and HIPAA compliance.

Position Summary

Ideal Home Health Agency is seeking a compassionate, dependable, and organized Part-Time Registered Nurse (RN) to join our home health clinical team. The RN will complete admissions, recertifications, supervisory visits, and other required nursing visits while providing clinical oversight of Home Health Aides.

This position offers a flexible schedule and the opportunity to provide individualized care to patients in their homes while working closely with caregivers, physicians, and agency staff.

Key Responsibilities

• Complete Start of Care/Admission assessments, recertifications, supervisory visits, and other required nursing visits.
• Assess patients' medical, functional, psychosocial, and environmental needs.
• Develop, review, and update individualized Plans of Care and ensure services are provided according to physician orders.
• Conduct Home Health Aide supervisory visits, evaluate caregiver performance, and provide education or coaching as needed.
• Monitor patient condition and promptly communicate significant changes to physicians and appropriate agency staff.
• Provide patient and caregiver education regarding medications, safety, infection prevention, disease management, and the Plan of Care.
• Complete accurate and timely clinical documentation in accordance with agency, payor, and regulatory requirements.
• Coordinate care with physicians, caregivers, office staff, and other members of the interdisciplinary team.
• Review caregiver compliance with the Plan of Care and address clinical or quality concerns.
• Participate in chart reviews, quality improvement, compliance activities, meetings, and required trainings.
• Maintain patient confidentiality and comply with HIPAA and applicable state and federal requirements.
