Suncrest Hospice seeks a Registered Nurse (RN) to provide patient care across home and facility settings. You will join an interdisciplinary team delivering psych-social assessments and hospice services. The role may involve Case Management, Admissions, or Resource responsibilities.
The position requires active RN licensure, ability to travel, and strong communication and organizational skills. You will work within a compassionate team and contribute to high-quality patient care.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
