Job summary

St. Joseph's Home Health, Inc. seeks a compassionate Registered Nurse to visit patients in their homes around Fort Stockton.

You will perform initial and ongoing assessments, monitor vital signs, review medications, and administer prescribed treatments while ensuring safety and comfort. Educate patients and families on home health care, coordinate with physicians and care teams, and maintain accurate charts.

Qualifications

• RN license required.
• CPR certification required.
• Nursing degree from an accredited program (Diploma/Associate/Bachelor).
• Bilingual preferred.
• Flexible schedule and ability to travel locally.

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs.
• Assess patients' conditions, chart observations, and perform vital signs and medication review.
• Administer medications as prescribed by the physician.

Responsibilities

• Visit patients in their homes for one-on-one visits

• Assess patients' conditions and chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medication as prescribed by the patient's physician
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient's quality of life
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Qualifications

• Graduate of an accredited Diploma, Associate, or bachelor's School of Nursing, or vocational nursing
• Current state license as an RN
• Proof of current CPR certification
• Compassionate, friendly demeanor
• Excellent communication skills
• Current driver's license and reliable transportation
• Bilingual preferred.
