Hospice of Virginia is hiring a RN Case Manager to support Forest and surrounding counties. Join our compassionate team of clinicians making a difference in end-of-life care. We offer comprehensive benefits including PTO, 401K with match, and competitive salaries.

Shift information: Monday–Friday, 8a–5p, with a rotating on-call. The RN Case Manager will oversee patient and family caseload, coordinate care with the interdisciplinary team, and ensure timely documentation and quality of care.

Qualifikationen

• Graduate from an accredited registered nursing program and be licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance is required.

Aufgaben

• Assume primary responsibility for patient and family caseload including the assessment, determining eligibility, admission processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data and assess information from patient factors that may impact needs and initiate intervention by other team members.
• Initiate communication with the attending physician, interdisciplinary team and other agencies for coordinating optimal care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes and timely care plans.
• Supervise and document delivery of care by the Home Health Aide as required by guidelines.

What Youll Do

• Assume primary responsibility for patient and family caseload including the assessment determining eligibility admission processing planning implementation and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical psychological social and spiritual factors that may impact the patient and familys needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re assess the patients needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records charts progress notes reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal State and Company guidelines
