Job summary

Affirmed Home Care, Inc. is seeking a compassionate RN to provide high-quality one-on-one in-home care across Elmwood and South Jersey. You will assess health needs, administer medications, perform skilled nursing tasks, and coordinate with families and physicians to ensure excellent outcomes.

Qualifications

• Active New Jersey RN license required.
• Minimum 1 year nursing experience preferred.
• Current CPR/BLS certification required.
• Strong clinical assessment and communication skills necessary.
• Reliable, compassionate, and professional demeanor essential.
• Comfortable working independently in a home-care setting.
• Valid driver’s license and reliable transportation preferred.
• Experience with EHR and clinical documentation preferred.

Responsibilities

• Provide skilled nursing care per client care plan.
• Assess and monitor health conditions and needs.
• Administer medications and treatments as prescribed.
• Perform wound, catheter care and other procedures within scope.
• Recognize and report changes in client condition.
• Coordinate care with physicians, families, and healthcare team.
• Maintain accurate, timely clinical documentation.
• Educate clients and families on medications and care plans.
• Promote client comfort, safety, dignity, and independence.
• Deliver professional care in the client’s home.

What You’ll Do
