Job summary

Complete Care Management is hiring an RN Registered Nurse to provide skilled nursing care to residents recently discharged from hospital or requiring long-term care.

The role includes administering medications, wound care, assessments, care planning, and collaboration with a multi-disciplinary team to ensure high-quality, patient-centered care.

Qualifications

• Licensed to practice as an RN in Delaware.
• Ability to pass background check, drug screen, and references.
• Demonstrated leadership and clinical skills.

Responsibilities

• Administer medications (oral, topical, IV, injectable).
• Provide wound care and pain management; apply dressings.
• Conduct health assessments and monitor changes; analyze data.
• Develop, update and implement individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and expected care.
• Maintain detailed documentation of health status, care provided and progress.
• Oversee and delegate tasks to LPNs and CNAs to ensure quality care.

Administer Medications: Give oral, topical, IV, injectable medications

Treatments: Perform wound care/prevention, manage pain, apply dressings

Assessment & Monitoring: Conduct health assessments, track changes & analyze data

Care Planning: Create, update & execute individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate to LPN's and CNA's ensuring quality care/teamwork
