Position Summary
Siena Woods Care Center is a full-service nursing facility specializing in complex care. We are committed to providing high-quality clinical and rehabilitative services in a compassionate, supportive environment designed to meet the evolving needs of our patients and residents.
RNs are responsible for providing compassionate, high-quality care to residents while ensuring compliance with all regulatory requirements. This role involves direct clinical care, documentation, communication with interdisciplinary teams, and supervision of nursing staff.
Resident Care & Clinical Duties
- Conductsdaily roundsto assess residents, verify hand-off information, and ensure continuity of care.
- Performscomprehensive assessmentsfor new admissions and residents with changes in condition.
- Delivers and overseesdirect nursing care, including:
- Administering medications.
- Recording vital signs and intake/output.
- Utilizing medical equipment and supplies as needed.
- Assists in thedevelopment, revision, and monitoringof resident care plans.
- Ensures compliance withregulatory standardswhile supporting resident well-being, physically, socially, and emotionally.
Communication & Documentation
- Notifies appropriate parties (physicians, families/legal reps, staff) of significant resident changes or incidents.
- Maintainsaccurate and timely documentationof care, assessments, and any unusual events.
- Escalates relevant concerns to theDirector of Nursingincluding regulatory issues, behavioral changes, and staffing challenges.
Staff Supervision & Development
- Directly supervisesnursing personnel by:
- Assigning duties and shifts.
- Evaluating staff performance.
- Training and onboarding new or temporary team members.
- Identifies staff development opportunities and providesreal-time coachingto support clinical excellence.
Regulatory & Safety Compliance
- Assists insurvey preparationand responds to audit-related activities.
- Enforces compliance withOSHA, bloodborne pathogen, and hazard communication protocols.
- Participates inemergency preparednessincluding fire and disaster drills.
Quality Assurance & Program Development
- Participates inquality assurance programsto maintain and improve care standards.
- Helpsreview and update facility policies and proceduresin accordance with regulations and best practices.
- Stays current throughcontinuing educationand mandatory in-service training.

- Proven ability to supervise, lead, and support nursing staff.
- Physical ability to liftup to 75 lbsand perform other essential duties.

Supervisory Responsibilities:
- Directly supervises licensed and unlicensed nursing staff.
- Indirectly oversees other care team members as related to resident care.
- Participates in hiring, orientation, performance reviews, scheduling, and resolution of personnel issues.
Job Requirements:
Education & Experience
- Graduate of anaccredited nursing program.
- Minimum1 year RN experiencepreferred."
