Job summary

Signature HealthCARE in Danville, KY is seeking a Full Time Registered Nurse for a day shift (7:00 AM – 7:00 PM) to deliver direct patient care in a 106-bed facility offering short-term rehabilitation and long-term care. A $10,000 sign-on bonus is available and pay is commensurate with licensed experience and shift differential.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Excellent verbal and written communication; ability to work independently and prioritize tasks.
• Familiarity with EHR systems and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs while ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides per policy.
• Monitor residents’ status and report changes to physician.
• Develop and review individualized care plans with the team.
• Document accurately per policy and procedures.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and regulatory surveys.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.
