Job summary

CVP is seeking a Registered Nurse to join the Dayton VA Medical Center team in Dayton, OH. The role involves delivering high-quality primary and specialty nursing care Monday to Friday, 8:00 AM–4:30 PM, across multiple units including ICU, ED, and Medical-Surgical settings.

You will document care in the EMR, administer medications, and participate in discharge planning. Ideal candidates hold an active Ohio RN license, BLS/ACLS, and a BSN is preferred.

• Must be eligible for Public Trust; US Citizenship required.
• Active, unrestricted Ohio nursing license; Ohio licensure before start date.
• Minimum 1 year direct patient care in last 3 years; VA experience preferred.

Responsibilities

• Provide direct and indirect nursing care across Primary Care, ICU, Med-Surg, Telemetry and ED as assigned.
• Document plan of care and all nursing actions in the EMR by end of each shift.
• Administer medications and IV therapies with 100% accuracy using BCMA.
• Assess patient condition and develop individualized care plans; reassess and adjust as needed.
• Respond to emergencies, initiate life-saving measures, and participate in discharge planning.
• Interpret cardiac rhythms and operate defibrillators and cardiac monitors as required.

Overview

CVP is seeking Registered Nurses to join our team in providing our Nation's Veterans with high-quality primary care and specialty nursing services at the Dayton VA Medical Center located at 4100 W Third Street, Dayton, OH 45428.

Working hours are Monday through Friday, 8:00 AM to 4:30 PM.

Responsibilities

• Provide indirect and direct nursing care by contributing to patient assessment, determining priorities, identifying nursing measures and therapeutic objectives, and evaluating outcomes across Primary Care, Intensive Care, Medical-Surgical, Telemetry, Cardiac Monitoring, and Emergency Room settings as assigned
• Perform critical care nursing to Intensive Care Unit patients as assigned
• Perform scrub and circulating duties in the ED and Operating Room as assigned
• Obtain patient history and pertinent family history
• Assess patient condition and develop an appropriate, individualized plan of care
• Reassess patient condition and revise plans of care based on identified nursing problems
• Document plan of care and all care given accurately in the patient's electronic medical record (EMR) prior to end of each shift
• Verify transcription of physician orders; no verbal orders are to be accepted except in compliance with facility policy
• Communicate pertinent patient data to charge nurse and/or physician as appropriate
• Administer medications and Intravenous (IV) therapies, ensuring a 100% accuracy rate using Bar Code Medication Administration (BCMA) in applicable areas
• Assess and document patient response to administered medications
• Interpret cardiac rhythms and respond to their clinical implications
• Operate defibrillators, EKG machines, cardiac monitors, and other emergency technical equipment
• Perform and manage central line and ventilator care in compliance with facility policy
• Perform suctioning including oral, nasogastric, and tracheostomy as needed
• Assess for and administer tube feedings as indicated
• Assist with routine urinary and bowel elimination including catheters, colostomies, and digital stimulation
• Perform wound care including dressing changes (sterile and non-sterile) and obtain wound cultures as necessary
• Perform skin assessments and initiate measures to prevent Hospital Acquired Pressure Ulcers (HAPU)
• Perform Fall Risk Assessment and implement recommendations and interventions to decrease the risk of falls and falls with injuries
• Perform Braden Scale assessments to measure risk for skin injuries and provide appropriate interventions
• Monitor and assess intake and output every shift, and more frequently as indicated
• Review all laboratory data and electronic medical records and act on findings appropriately
• Instruct patients and families regarding prescribed medications, therapies, and surgical and diagnostic procedures
• Assess patient and family knowledge level, implement plan of instruction, and record demonstrated level of understanding
• Participate in discharge planning as appropriate
• Be able to initiate life-saving measures in the absence of a physician
• Recognize and initiate appropriate measures in emergencies and assist with surgical and diagnostic procedures
• Care for patients with a variety of catheters and prostheses
• Ensure medication orders from the previous shift have been noted and either carried out or transcribed onto the medical records appropriately
• Document all care using the current Electronic Health Record (EHR) system; documentation and coding functions must be completed by end of each patient care encounter
• Report all patient safety incidents using the VA Patient Safety Reporting System within 24 hours of the incident
• Report any patient complaints to the COR or designee within 24 hours
• Comply with all VA patient safety and infection control standards of care, making every effort to prevent medication errors, falls, and patient injury
• Comply with all HIPAA, Privacy Act, and VA confidentiality requirements; maintain privacy and confidentiality of all patient information at all times

Qualifications

• Must be eligible to obtain a Public Trust government security clearance; US Citizenship is required
• Graduation from a school of professional nursing approved by the appropriate State agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE) at the time the program was completed; a Bachelor of Science in Nursing (BSN) is preferred
• Current, full, active, and unrestricted license to practice as a Registered Nurse issued by the Ohio Board of Nursing (OBN); licensure in any US state or territory will be considered, however Ohio licensure must be obtained prior to start date
• Minimum of one (1) year of direct patient care experience within the last three (3) years; VA experience is preferred
• Experience in one or more of the following specialty areas is preferred: Primary Care, Intensive Care/Critical Care, Medical-Surgical, Telemetry/Cardiac Monitoring, Emergency Room
• Current BLS certification from an American Heart Association-approved vendor
• Current ACLS certification from an American Heart Association-approved vendor
• Current CPR certification from an American Heart Association-approved vendor
• Proficiency in Electronic Health Record (EHR) systems; training on VA-specific systems including VistA and CPRS will be provided upon onboarding
• Familiarity with medical terminology, hospital policies and procedures, and clinical documentation standards
• Ability to speak, understand, read, and write English fluently
