Job summary

Aptive is hiring Licensed Practical Nurses to support direct patient care delivery within the Primary Care PACT team at the Dayton VA Medical Center. The role demands obtaining patient histories, assisting with medication, and performing assessments.

The ideal candidate will be a graduate of an accredited program, hold a current Ohio LPN license, and have significant patient care experience. This is a full-time, on-site position with no federal holidays.

Qualifications

• Minimum 1 year direct patient care within the last 3 years.
• Must pass background investigation.
• Completion of required VA mandatory training prior to start.

Responsibilities

• Obtain patient histories and assess nursing care needs.
• Assist with medication administration and document in EHR.
• Perform skin assessments and preventive care interventions.
• Carry out physician orders and document in patient medical record.
• Support emergency response and initiate life-saving measures as needed.

Location: On-Site | Dayton VA Medical Center, 4100 W. 3rd St., Dayton, OH 45417
Schedule: Monday–Friday, 8:00am–4:30pm | No federal holidays
Employment Type: Full-Time Contract
Contract Period: 8/1/2026–7/31/2027 (+ Option Year)

Primary Responsibilities

Responsibilities:

• Obtain patient histories and assess nursing care needs
