Job summary

Medix is seeking a Registered Nurse (RN) – Community Care in Dayton to support Veterans through non-VA community healthcare providers. The RN will serve as a clinical bridge among the VA, Veterans, and external providers, coordinating care, reviewing records, triaging referrals, and ensuring timely, high-quality patient care in line with VA guidelines.

Qualifications

• Graduate of a state-approved professional nursing program (ACEN or CCNE accredited).
• Active, unrestricted RN license with Ohio Board of Nursing.
• 1 year of direct patient care or clinical care management in last 3 years.
• Current CPR certification (AHA).
• Current BLS certification.
• Ability to complete Tier 1 / Low Risk background investigation.
• Ability to obtain PIV card and immunizations with TB clearance.
• Completion of VA training including EHR, Privacy/HIPAA, Cyber Security, Active Threat, and Workplace Harassment prevention.

Position Overview

We are seeking a Registered Nurse (RN) – Community Care to support Veterans receiving medical services through non-VA community healthcare providers.

The Community Care RN serves as a clinical bridge between the VA, Veterans, and community healthcare providers, coordinating care, reviewing clinical information, triaging referrals, and ensuring timely, safe, and high-quality patient care in accordance with VA guidelines and community care processes.

Key Responsibilities

Review and clinically assess community care consults and referrals for appropriateness.

Triage incoming requests and determine the appropriate level of care within the scope of nursing practice.

Coordinate care between VA healthcare teams, Veterans, and external specialists, diagnostic centers, and hospitals.

Communicate clinical expectations, care plans, and follow-up requirements to community providers.

Review medical records, lab results, imaging reports, treatment documentation, and hospital discharge summaries received from community providers.

Integrate and document external medical records within the VA Electronic Health Record (EHR) system.

Review community provider treatment orders, prescriptions, and recommended follow-up care.

Coordinate with VA pharmacy and primary care teams to support safe medication reconciliation and continuity of care.

Conduct telephone consultations with Veterans regarding appointments, treatment plans, procedures, and follow-up care.

Assess Veteran status and provide appropriate education and instructions to Veterans and families regarding prescribed care.

Maintain accurate documentation of patient encounters and clinical activities in the EHR.

Follow infection control, patient safety, and quality assurance standards.

Report patient safety incidents within 24 hours through the JPSR system.

Participate in peer review, quality improvement, and other clinical quality initiatives.

Maintain compliance with applicable VA policies, procedures, and required training.

Required Qualifications

Graduate of a state-approved professional nursing program accredited by ACEN or CCNE.

Active, unrestricted Registered Nurse (RN) license issued by the Ohio Board of Nursing (OBN).

Minimum of 1 year of direct patient care or clinical care management experience within the last 3 years.

Current CPR certification through the American Heart Association (AHA).

Current BLS certification.

Ability to successfully complete a Tier 1 / Low Risk background investigation.

Ability to obtain and maintain a PIV card.

Completion of required immunizations and TB clearance.

Ability to complete required annual VA training, including:

VA Electronic Health Record (EHR)

VA Privacy/HIPAA

Cyber Security

Active Threat

Prevention of Workplace Harassment

Preferred Qualifications

Bachelor of Science in Nursing (BSN) preferred, but not required.

Previous VA healthcare experience preferred, but not required.

Experience coordinating care across multiple healthcare organizations is a plus.

Strong clinical judgment, communication, organization, and patient advocacy skills.
