• Educate patients, families, and caregivers on disease management, self-care techniques, and rehabilitation strategies.
• Participate in quality improvement initiatives and patient satisfaction surveys to enhance the overall patient experience.
• Maintain accurate and detailed records of patient care and progress.
• Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.

Weekends: 6:30pm-7:00am with baylor plan
