Job summary

BrightStar Care is expanding in the Burnsville and surrounding areas and is seeking a Registered Nurse (RN) to serve as a Home Care Case Manager four or five days per week. Four days a week is salaried at $65,000 and five is $82,000.

This salaried role works closely with a team of RNs and LPNs to complete assessments, reassessments, and select skilled nursing tasks for private‑pay and veteran clients. As a non‑Medicare agency, BrightStar Care does not require OASIS documentation.

Qualifications

• Active RN license in Minnesota.
• Previous experience in a home health or acute care setting is advantageous.
• Exceptional interpersonal and communication skills.
• Committed to delivering high-quality, patient-centered care.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
• Collaborate with multidisciplinary team members to optimize patient care and outcomes.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure accurate documentation and compliance with all regulatory guidelines.

As a non‑Medicare agency, BrightStar Care does not require OASIS documentation. Our nursing team is collaborative, supportive, and focused on helping clients remain safe and comfortable in their homes. This position offers PTO accrual starting on day one and paid mileage between client visits and the office. Most clients will be located within the Burnsville area (Eagan, Inver Grove Heights, Apple Valley, and surrounding communities), allowing you to work close to home.
