Qualifications

• Current Indiana RN license is required.

• Long-Term Care, Skilled Nursing, Rehabilitation experience preferred but not required.

Responsibilities

• Provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting.
• Assess, monitor, and document resident conditions and changes.
• Administer medications and treatments according to physician orders.
• Collaborate with physicians, nurse practitioners, therapists, and interdisciplina ry team members.
• Assist with admissions, discharges, care planning, and resident rounds.
• Maintain accurate nursing documentation and medical records.
• Ensure compliance with state, federal, and facility regulations.
• Respond to resident and family concerns with professionalism and compassion.
