Job summary

KALESTA HEALTHCARE GROUP is looking for a dedicated Registered Nurse to join our Bellevue Post-Acute team. You will play a crucial role in assessing residents' health, developing individualized care plans, and ensuring high-quality patient care in a skilled nursing facility.

Qualifications

  • Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Current active Washington State RN license.
  • Experience in skilled nursing or long-term care preferred, but new graduates welcome.

Responsibilities

  • Assess residents’ health status and develop care plans.
  • Administer medications and treatments as per orders.
  • Document changes in residents’ conditions and communicate with the care team.
  • Educate residents and families on health management.
  • Participate in care planning conferences.

Reporting

This position reports directly to the Director of Nursing and collaborates with the interdisciplinary care team.

Key Responsibilities
  • Assess residents’ physical and mental health status and develop individualized care plans.
  • Administer medications and treatments safely according to physician orders and facility protocols.
  • Monitor, record, and communicate changes in residents’ conditions to the care team.
  • Educate residents and families on care plans, health management, and preventive strategies.
  • Participate in interdisciplinary care planning and resident care conferences.
  • Maintain accurate and timely documentation of resident care, interventions, and outcomes.
  • Ensure compliance with infection control practices, safety standards, and resident rights.
  • Provide mentorship and guidance to licensed and unlicensed nursing staff.
  • Participate in facility quality improvement initiatives, in‑services, and continuing education programs.
Education and License Requirements
  • Graduate of an accredited nursing program (Associate’s or Bachelor’s degree in Nursing).
  • Current, active Washington State RN license.
Experience

Experience in skilled nursing, long‑term care, or rehabilitation settings preferred, but new graduates are welcome to apply.

Working Conditions
  • Skilled Nursing Facility environment with direct interaction with residents, staff, and visitors.
  • May involve walking, standing, lifting, and exposure to bodily fluids and infectious materials.
  • Must be able to cope with the stresses of working in a 24‑hour care environment serving elderly and medically complex residents.
