Qualifications

• Graduate of an accredited nursing school.
• Associate degree required; BSN preferred.
• Current Connecticut RN license.
• BLS certification by the American Heart Association.
• Excellent assessment and critical thinking skills.

Responsibilities

• Provides direct nursing care in accordance with established policies, procedures and protocols.
• Implements and monitors patient care plans; monitors, records and communicates patient condition.
• Serves as a primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates patient's discharge planning needs with the care team.
• Demonstrates ability to identify patient needs across physical, psychosocial, cultural, spiritual, and age-related aspects.
• Delegates care tasks to nursing staff appropriate to their level of education.
• Participates in daily interdisciplinary rounds and provides input for patient outcomes.
• Follows ANA standards and code of ethics in practice; engages in professional growth.
• Develops, implements and revises individualized plans of care.
• Performs other duties as assigned.
