Job summary

Colorado Coalition for the Homeless seeks a Nurse Case Manager to deliver community-based nursing care to clients living on the streets and in supportive housing.

This field-based role collaborates with an integrated health team, supports health outreach, and uses trauma-informed, harm-reduction approaches; bilingual skills preferred.

The role includes triage, education, medication administration, and mentoring; scheduling may include day, evening, or weekend clinics.

Qualifications

  • Associates, Bachelors or Masters degree in Nursing from an accredited college or university.
  • Current Registered Nurse licensure in the State of Colorado.
  • Basic Life Support certification.
  • Experience in ambulatory, public, community, mental health, emergency, SUD, or hospice nursing is preferred.

Responsibilities

  • Provide skilled nursing care according to community standards and CCH protocols.
  • Conduct rapid triage assessments and provide appropriate care recommendations.
  • Educates and empowers patients by providing health education addressing chronic and acute conditions.
  • Administer medications safely and effectively as per protocols.
  • Collaborate closely with a multidisciplinary healthcare team to deliver holistic patient care.
  • Facilitate access to internal programs such as health outreach and The Stout Street Health Center and Recuperative Care Center.
  • Oversee care for patients with transitional, chronic, and complex health conditions.
  • Mentor new nurses and nursing students, enhancing their clinical skills and confidence.
  • Engage in continuous professional development and participate in quality improvement initiatives.
  • Delivers comprehensive patient-centered care within an integrated, trauma-informed, harm-reduction model.

The Nurse Case Manager primarily provides services in the community within the framework of the nursing scope, professional nursing ethics, and state and federal laws and regulations. This is a field-based position with most services provided to patients in settings outside primary health center sites. The Nurse Case Manager collaborates with other integrated health and supportive housing service team members to deliver care to clients living on the streets and in supportive housing communities.

Duties/Responsibilities
  • Provide skilled nursing care according to community standards and CCH protocols.
  • Conduct rapid and accurate triage assessments, providing appropriate care recommendations.
  • Educates and empowers patients by providing appropriate health education and information, addressing common chronic and acute health conditions such as diabetes management, hypertension management, nutrition, wound care, infectious disease, gender-affirming care, family planning including parenting, adoption, abortion, and disease prevention.
  • Administer medications safely and effectively as per protocols.
  • Collaborate closely with a multidisciplinary healthcare team to ensure holistic patient care.
  • Facilitate access to internal programs such as health outreach, The Stout Street Health Center, and The Stout Street Recuperative Care Center.
  • Oversee care for patients with transitional, chronic, and complex health conditions.
  • Mentor new nurses and nursing students, enhancing their clinical skills and confidence.
  • Engage in continuous professional development and participate in quality improvement initiatives.
  • Delivers comprehensive patient-centered care within an integrated, trauma-informed, culturally aware, harm-reduction model, addressing the physical, emotional, and social needs of patients.
  • Ability to perform quick and accurate triage assessments with appropriate recommendations of care as outlined in SSHC nursing protocols via telephone or in-person.
  • Collaborate with other members of the multidisciplinary health care team and coordinate care with internal programs.
  • As a member of an OBH licensed Substance Use Disorder (SUD) treatment team, provide treatment services such as case management, crisis services, initial intake program screenings, individual medication-assisted treatment (MAT) counseling, and administering scales to assess withdrawal.
  • Provide nursing services as needed to all CCH programs and facilities.
  • Ability to work day, evening, or weekend clinics as scheduled or appropriate.
Education
  • Associates, Bachelors or Masters degree in Nursing from an accredited college or university.
  • Current Registered Nurse licensure in the State of Colorado.
  • Basic Life Support certification.
Experience
  • Experience in ambulatory, public, community, mental health, emergency, SUD, or hospice nursing is prefered.
  • Familiarity with diverse or marginalized communities are highly desirable.
  • Bilingual (Spanish/English) preferred.
