Responsibilities:

1. Provide comprehensive nursing care in clients' homes under RN supervision.
2. Develop care plans with interdisciplinary team members and report to Supervisors.
3. Educate patients and caregivers on health matters.
4. Accept and document physician orders, obtaining co-signatures as required.
5. Follow up with physicians, clients, and families regarding client issues.
6. Maintain accurate documentation of care provided.

Minimum Requirements:

• Active nurse license in Connecticut or a compact license.
• High School Diploma or GED.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• At least 6 months of RN Pediatric experience preferred.
• Home Health experience preferred.
• Reliable transportation.
• Commitment to quality patient care.
• Ability to sit, stand, bend, lift, and move intermittently, up to 50 lbs.

You will report to the Clinical Team Manager.
