Job summary

Qualifications

• English language proficiency as required by VA standards.
• Graduate of an approved nursing program and board-eligible/licensed to practice nursing in the United States.
• State license to practice nursing (full, active, unrestricted).
• Preferred experience in direct patient care and care coordination.

Responsibilities

• Provide direct patient care and supervise direct patient care activities.
• Coordinate care through the patient care team.
• Educate patients on self-management and wellness.
• Participate in quality improvement and outcomes utilization.
• Collaborate with interdisciplinary team to improve patient outcomes.
