Job summary

Primary Care Solutions is seeking a Registered Nurse to support Veterans within a Patient Aligned Care Team (PACT). The role includes direct patient care, care planning, and coordination of services for optimal outcomes.

You will work onsite with occasional remote elements in a hybrid schedule, assisting practitioners, triaging urgent needs, and educating patients on wellness. A valid RN license and BLS certification are required, with 1 year in primary care preferred.

Qualifications

• 1 year of Primary Care and/or Chronic Disease Management experience.
• Current, active, and unrestricted state Nursing License.
• Strong computer skills, including EMR experience.

Responsibilities

• Provide healthcare of the highest quality to America’s Veterans as part of a PACT.
• Assist practitioners with patient care and nursing skills in patient assessment.
• Triage urgent/emergent same-day patients for the team and identify high acuity needs.
• Coordinate patient care and navigate the care system for patients and families.
• Participate in daily team huddles and communication.
• Respond to patient calls, messages, and alerts.
• Educate patients and families on self-management, prevention, and wellness.
• Travel up to 50% as required.

RESPONSIBILITIES:

As a Registered Nurse with Primary Care Solutions, you will enjoy the privilege of providing healthcare of the highest quality to America’s Veterans as part of a Patient Aligned Care Team (PACT) and will have an active role in developing and implementing a patient care plan.

• Office hours are Mon - Fri 8:00am to 4:30pm.
• This is a Full-Time position onsite & remote (2 weeks onsite/2 weeks remote)
• Assist practitioners with patient care and practice specialized nursing skills in patient assessment
• Triage urgent/emergent same-day patients for the team. Identifying acute patient needs and high acuity patients, identifying Veterans at risk for uncoordinated care, and mobilizing a care team for optimal patient outcomes
• Provide support by assisting our patients in navigating and coordinating their care
• Participate in a daily team huddle
• Answer and returns patient telephone calls, messages and alerts
• Provide patient and family education focused on self-management, prevention, and wellness
• Monitor discharge list and contact patients for follow up
• Establish and maintain a warm, welcoming, and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met
• Promote achievement of clinical goals and metrics
• Ability to travel 50% of the time

JOB REQUIREMENTS:

• 1 year of Primary Care and/or Chronic Disease Management experience
• Current, active, and unrestricted state Nursing License
• Current BLS for providers from the American Heart Association (to be renewed annually)
• Strong computer skills, including EMR experience.
