Job summary

Primary Care Solutions based in Ada, Oklahoma is looking for a Full-Time Nurse. The position involves assisting with patient care, managing urgent requests, and educating patients about self-management and wellness.

Candidates should have at least 1 year of experience in Primary Care or Chronic Disease Management, a current Nursing License, and BLS certification. The role is onsite with a welcoming team dedicated to delivering outstanding care to Veterans.

Qualifications

• 1 year of Primary Care and/or Chronic Disease Management experience.
• Current, active, and unrestricted state Nursing License.
• Current BLS for providers from the American Heart Association.

Responsibilities

• Assist practitioners with patient care using specialized nursing skills.
• Triage urgent/emergent same-day patients for the team.
• Provide patient and family education focused on self-management.
• Monitor discharge list and contact patients for follow-up.

Primary Care Solutions provides industry-leading Primary Care services to Veterans. Veteran-led and clinician-managed, we deliver compassionate, expert medical care, establishing personal bonds with our Veterans in our Community-Based Outpatient Clinics (CBOC’s). Responsibilities

• Office hours are Mon - Fri 8:00am to 4:30pm.
• This is a Full-Time position onsite. (no remote options)
• Assist practitioners with patient care and practice specialized nursing skills in patient assessment
• Triage urgent/emergent same-day patients for the team. Identifying acute patient needs and high acuity patients, identifying Veterans at risk for uncoordinated care, and mobilizing a care team for optimal patient outcomes
• Provide support by assisting our patients in navigating and coordinating their care
• Participate in a daily team huddle
• Answer and return patient telephone calls, messages and alerts
• Provide patient and family education focused on self-management, prevention, and wellness
• Monitor discharge list and contact patients for follow up
• Establish and maintain a warm, welcoming, and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met
• Promote achievement of clinical goals and metrics

Job Requirements

• 1 year of Primary Care and/or Chronic Disease Management experience
• Current, active, and unrestricted state Nursing License
• Current BLS for providers from the American Heart Association (to be renewed annually)
• Strong computer skills, including EMR experience.
