Job summary

Primary Care Solutions in Greenville, Texas, is seeking a Registered Nurse to join a Veterans-focused care team on a full-time, on-site basis. You will work in a Patient Aligned Care Team (PACT) to develop and implement patient care plans and deliver high-quality medical services to Veterans.

The position requires an active RN license, BLS certification, and at least 1 year of Primary Care/Chronic Disease Management experience.

Qualifications

• Active RN license in state of practice.
• BLS for Providers (American Heart Association) current.
• Minimum 1 year of Primary Care and/or Chronic Disease Management experience.
• Proficient with electronic medical records (EMR).

Responsibilities

• Provide high-quality healthcare to Veterans within a Patient Aligned Care Team (PACT).
• Assist practitioners with patient care and nursing assessments.
• Triage urgent and same-day patients for the care team.
• Support patients in navigating and coordinating their care.
• Participate in daily team huddles and care planning.
• Respond to patient calls, messages, and alerts.
• Educate patients and families on self-management, prevention, and wellness.
• Monitor discharge lists and arrange follow-up care.
• Maintain a warm, welcoming environment for patients and colleagues.
• Help achieve clinical goals and performance metrics.

RESPONSIBILITIES:

As a Registered Nurse with Primary Care Solutions, you will enjoy the privilege of providing healthcare of the highest quality to America’s Veterans as part of a Patient Aligned Care Team (PACT) and will have an active role in developing and implementing a patient care plan.

• Office hours are Mon - Fri 8:00am to 4:30pm.
• This is a Full-Time position onsite. (no remote options)
• Assist practitioners with patient care and practice specialized nursing skills in patient assessment
• Triage urgent/emergent same-day patients for the team. Identifying acute patient needs and high acuity patients, identifying Veterans at risk for uncoordinated care, and mobilizing a care team for optimal patient outcomes
• Provide support by assisting our patients in navigating and coordinating their care
• Participate in a daily team huddle
• Answer and returns patient telephone calls, messages and alerts
• Provide patient and family education focused on self-management, prevention, and wellness
• Monitor discharge list and contact patients for follow up
• Establish and maintain a warm, welcoming, and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met
• Promote achievement of clinical goals and metrics

JOB REQUIREMENTS:

• 1 year of Primary Care and/or Chronic Disease Management experience
• Current, active, and unrestricted state Nursing License
• Current BLS for providers from the American Heart Association (to be renewed annually)
• Strong computer skills, including EMR experience.
