UChicago Medicine AdventHealth is seeking a Registered Nurse to assess, plan, and implement patient care with an interdisciplinary team. The role emphasizes safety, education, and holistic care across diverse patient populations.

The position works full-time during the day in Chicago, IL, with responsibilities including care coordination, documentation, and patient-family education. Bilingual English/Spanish is preferred to support a broad patient base.

Qualifications

• Associate's of Nursing is required.
• Bachelor's of Nursing preferred.
• Registered Nurse (RN) license required.
• BLS certification required.

Responsibilities

• Assess patients' physical, psychosocial, cultural, and spiritual needs.
• Develop and implements patient care plans with the interdisciplinary team.
• Administer IVs, wound care, medications, and catheterization as needed.
• Advocate for patient safety and rights; ensure accurate documentation.
• Educate patients and families; support self-management and teach-back methods.
• Coordinate high-risk patient group visits and care transitions.
• Contribute to patient experience and satisfaction initiatives.

Job Description

Schedule: Monday - Friday 8:30am-5pm Bilingual English/Spanish preferred. Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data. Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards. Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection. Advocates for and protects the health, safety, and rights of each patient. Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching. Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology. Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity. Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy. Other duties as assigned. Coordinates and implements group visits for identified high-risk patient populations. Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services. Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
