Qualifications

• Graduation from an accredited nursing program with a baccalaureate degree or RN experience.

This role is for an experienced Registered Nurse (RN) within Primary Care, focusing on supporting patients with chronic conditions transitioning from hospitalization back to primary care.

Key Duties Include

• Conducting post-hospital follow-up calls for at-risk patients.
• Performing home visits for care coordination, medication reconciliation, symptom assessment, safety evaluation, and goal setting.
• Collaborating with primary care providers to develop and monitor individualized care plans.
• Managing a panel of patients through ongoing follow-up and care coordination.
• Providing patient education on chronic conditions, treatment plans, and medications to promote engagement and self-care.

The RN is responsible for coordinating nursing care, including direct patient care, education, and transitions of care, while demonstrating leadership, communication, and critical thinking skills.

Qualifications

• Graduation from an accredited nursing program with baccalaureate degree or relevant RN experience.
• Current RN license and Basic Life Support (BLS) certification as required by the state.
• Experience working in team settings, with strong communication and computer skills.
• Ability to work flexible hours and adapt to changing clinical situations.
