Primary Care Solutions, Inc. seeks a Nurse Practitioner to join our VA Community-Based Outpatient Clinic (CBOC). You will provide world‑class care to U.S. Veterans, leading patient management in collaboration with the PACT team.

The role emphasizes coordination, documentation in the VA system, and adherence to best practice standards. Candidates must hold an MSN, be board-certified, and possess active licensure and DEA/BLS as required.

Qualifications

• Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
• Board-certified in family practice or internal medicine.
• Current, unrestricted license in the state of employment.
• BLS for providers and DEA registration as applicable.

Responsibilities

• Examine the patient and determine the best course of action while documenting in VA VISTA/CPRS.
• Develop and implement patient management plans with PACT.
• Discuss outcomes, policies, and records with Medical Director.
• Complete clinical reminders to keep patient data up to date.

RESPONSIBILITIES:

As a Nurse Practitioner with Primary Care Solutions, you'll have the opportunity to serve our nation's Veterans with the same exceptional level of care our Veterans have shown while serving our country. We are seeking healthcare professionals who are driven and committed to ensuring the health and well-being of our nation's Veterans.

• Examine the patient/client and determine the best course of action while documenting in our paperless health program – The VA’s VISTA/CPRS electronic patient record system.
• Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Completes all clinical reminders to keep each patient up to date in the system.

JOB REQUIREMENTS:

• Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
• Board-certified in family practice or internal medicine
• Certification by an applicable professional organization
• 1 year of clinical medical practice preferred
• Current, unrestricted license in the state of desired employment
• Current BLS for providers from the American Heart Association (to be renewed annually)
• Current, unrestricted Drug Enforcement Administration (DEA) registration
• Excellent computer skills
