Job summary

Primary Care Solutions, Inc. seeks a skilled Nurse Practitioner to join our VA Community-Based Outpatient Clinic (CBOC).

You will deliver world-class care to US Veterans, lead patient management with the PACT team, and uphold the highest clinical standards in a supportive, mission-driven environment. Qualifications

• MSN degree required for Nurse Practitioner.
• Board-certified in family practice or internal medicine.
• Current, unrestricted license in the state of employment.
• BLS for providers (American Heart Association) current.
• DEA registration current.

Responsibilities

• Examine the patient and determine the best course of action, documenting in VA VISTA/CPRS.
• Develop and implement patient management plans with the PACT team.
• Discuss patient outcomes with Medical Director and align with standards of care.
• Complete clinical reminders to keep each patient up to date in the system.

RESPONSIBILITIES:

As a Nurse Practitioner with Primary Care Solutions, you'll have the opportunity to serve our nation's Veterans with the same exceptional level of care our Veterans have shown while serving our country. We are seeking healthcare professionals who are driven and committed to ensuring the health and well-being of our nation's Veterans.

• Examine the patient/client and determine the best course of action while documenting in our paperless health program – The VA’s VISTA/CPRS electronic patient record system.
• Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
• Discuss patient outcomes, policies, procedures, and records with Medical Director.
• Completes all clinical reminders to keep each patient up to date in the system.

JOB REQUIREMENTS:

• Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
• Board-certified in family practice or internal medicine
• Certification by an applicable professional organization
• 1 year of clinical medical practice preferred
• Current, unrestricted license in the state of desired employment
• Current BLS for providers from the American Heart Association (to be renewed annually)
• Current, unrestricted Drug Enforcement Administration (DEA) registration
• Excellent computer skills
