Registered Nurse

We are seeking Registered Nurses to join our Team! You will be responsible for the assessment, management, and treatment of assigned patients.

Responsibilities

• Visit clients in the privacy of their residence in designated geographic territories.
• Schedule and perform monthly supervisory visits.
• Schedule and perform Quarterly Evaluations.
• Complete timely and accurate clinical notes including addressing patient progress.
• Evaluate the client status and the skills of workers while the staff member is present.
• Keep documentation of all communication with clients, family members and support staff.
• Provide patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
• Counsel and educate the patient and family in meeting healthcare and related needs.
• Write 485/ Plan of Care for patients when reauthorizations are due for patients.
• Other projects/duties as assigned.

Requirements

• Home Care experience preferred
• Excellent verbal and written communication skills.
• Highly motivated, organizational, leadership and relationship building skills.
• Ability to work in a very detailed, fast paced environment.
• Currently holds a RN license in PA.
• Valid state driver’s license and car insurance.
