Job summary

Home Health Care, Inc. is seeking a Registered Nurse to join our team. You will assess, manage, and treat assigned patients in home settings across designated territories in Montgomeryville, PA.

Responsibilities include monthly supervisory visits, quarterly evaluations, and detailed clinical notes. You will educate patients and families, oversee staff performance, and prepare plans of care for reauthorizations. Equal opportunity employer.

• Counsels and educates the patient and family in meeting healthcare and related needs.
• Write 485/ Plan of Care for patients when reauthorizations are due for patients.
• Other projects/duties as assigned.

Requirements:

• Home Care experience preferred
• Excellent verbal and written communication skills.
• Highly motivated, organizational, leadership and relationship building skills
• Ability to work in a very detailed, fast paced environment
• Currently holds a RN license in PA
• Valid state driver’s license and car insurance
